1. Click on the Children button and select "Child Summary".
2. In the top, right-hand corner click on the green Medical box to open the Accidents and Incidents form.
Alternatively, click on "Misc. Details" and select "Accidents & Incidents" from the drop-down list.
To upload a file, click on "Choose File" and select the file from your device.
For body maps, click on "Create body map" and use the pen tool to indicate where on the child the injury is located. Choose between "Front and Back", "All Sides" or "Head".
Once you've selected your template, you can pick your line colour. Choose any colour you like from the provided palette and then indicate where on the child the injury was!
Once you are happy with your body map, click "Save" and then "Upload" to add it to your form.
Please note that only one file can be added to an Accident/Incident.
6. If you have accidentally attached the wrong file, simply click on the "Clear" button to remove it and repeat step 5.
7. If you would like for the Accident/Incident to appear in Parent Portal. Click on the "Yes" toggle beside "Show in Parent Portal". Once the parent/carer has acknowledged the accident, the "Parent Portal Acknowledgement" field will show you the carer name and date of acknowledgment.
8. Click on the "Save" button to complete the Accident/Incident record.
9. The Accident/Incident will now be added to the child's record.
10. You can view details of recorded Accidents/Incidents by clicking on the details icon on the right-hand side.

You can also print details of the accidents/incidents logged for a child by clicking on the 'Print' button
To edit an Accident or Incident, complete the following steps;
- Locate the required child and click on the Misc. Details tab, then select 'Accidents & Incidents' from the dropdown list.
- Click on the 'Details' button for the accident/incident you wish to edit.
- Details of the accident / incident will be displayed in the form below. For example:
4. If you want to view a copy of the attached file, click on the blue link to download it.
5. Make your changes and click on the 'Update' button.
To delete an accident / incident from a child's record, do the following:
- Repeat steps 1 - 3 above.
- Click on the 'Delete' button.
